Hospices have two new quality measures to begin collecting data for on April 1, 2017 - Hospice Visits When Death is Imminent Measure Pair and the Hospice and Palliative Care Composite Process Measure.  The HIS tool has been updated to reflect the new reporting requirements and hospices will begin using the HIS version 2.00.0 with admissions and discharges after April 1.  A Hospice Compare website and public reporting is on schedule for implementation in summer 2017.  To utilize standardized tools most successfully, it is important to understand the instructions for each component of the HIS and the measure definitions.  

This informative webinar will:

  • Review the current HIS reporting requirements
  • List the two new quality measure details
  • Explain the four new HIS items
  • Provide a preview of publicly reported data
  • Share common HIS questions and concerns while providing tips for dealing with these issues

Presenters:  Katie Wehri, BSBA, CHC, CHPC, is the hospice specialist with Ohio Council for Home Care & Hospice, has an extensive background in regulatory, licensing/certification and accreditation standards interpretation and implementation.  Certified in healthcare compliance (CHC) and healthcare privacy compliance (CHPC) through the Health Care Compliance Association, she also works in hospice operations with the National Association for Home Care & Hospice.  Katie has been consulting nationally in these areas since 2009.

Amy Hoyda, RN, BSN, MBA, CHPN, is the Director of Quality Assurance/Performance Improvement/Education at Bridge Home Health & Hospice in Findlay, OH.  Her career positions include post-acute care setting in various roles. She has experience to apply healthcare quality measure management and lean process improvement methodologies in the post-acute care setting with a special focus on educating about quality integration and lean application to both management and front line staff.  

Continuing Education:  The presenters and program planners have no conflict of interest in regard to this webinar.  RNs and LPNs must participate in the entire presentation in order for contact hours to be awarded.  To apply for nursing contact hours, please mail an evaluation form and a $10 processing fee for each individual requesting hours.  Additionally, send in a completed sign-in sheet, listing the individuals at your facility that participated and noting those requesting contact hours. 

Individuals requesting contact hours will receive a certificate of contact hours earned.  Please make sure that you enter the Contact Person’s name in the Registration Form.  All correspondence and site registration information will be sent to this person.  No commercial support has been provided for this program.

Ohio Council for Home Care and Hospice is an approved provider of continuing nursing education by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation.  (OBN-001-91) (OH -157, 4/1/2019).

Confirmation: A confirmation will be sent via email to verify site registration, as will the handouts, Internet link and 800 number for entering the webinar, and instructions for participation.  If your Contact Person does not receive the E-mail confirmation two (2) business days prior to the webinar, please contact our office to verify your status.

Registration Fee per site/phone connection: Registration fees are for one (1) phone connection.  SHARING OF REGISTRATION OR FEES WITH OTHER AGENCIES AND/OR INDIVIDUALS IS PROHIBITED.  The live webinar includes the opportunity to ask questions of the presenter.  Contact hours may be earned for listening to the audio recording up to 6 months following the live webinar.  If you desire to purchase both the live webinar and the audio recording, pay the registration fee and an additional $30.
